Center for Diabetes and Endocrinology – University of Maryland Medical Center (Midtown campus)

The University of Maryland Center for Diabetes and Endocrinology is dedicated to conquering diabetes in all of its forms, is the global leader in diabetes research, care and education. Its programs reflect a philosophy of a comprehensive team approach to diabetes treatment with programs designed to help children and adults with diabetes take charge of their own health and well-being.

The Clinical Pharmacist provides medication management in a collaborative practice, which includes diabetes mellitus and metabolic syndrome management, tobacco cessation, comprehensive medication therapy management and medication reconciliation to “Transitions of Care” patients.

P3 eHealth Services

The P3 eHealth Services is dedicated to providing innovative pharmacy practice, which encompasses medication management for high risk patients. In collaboration with key stakeholders, we provide comprehensive medication therapy management (CMTM), consultative services, and patient education. We deliver services in-person and through the use of telephonic and virtual technology. Our program reflects a philosophy of collaborative practice targeting culturally diverse patients, who are at high risk for poor health outcomes.

 The P3 eHealth Services promotes collaborations and interprofessional education to improve patient health outcomes by incorporating telemedicine technology.
