Dr. P. Tim Rocafort is an assistant professor in the Department of Pharmacy Practice and Science at the University of Maryland School of Pharmacy.

About Dr. Rocafort:

Dr. Rocafort primarily focuses on patient care in the community and ambulatory care setting. He currently practices at the Interprofessional Care Transition of Care Clinic at the University of Maryland Medical System at Prince George’s Hospital Center as a Clinical Pharmacist. Dr. Rocafort promotes effective transition to community-/ambulatory-based care of high-risk, high-utilizer patients, incorporation of medication therapy management services for continuity of care, and public health awareness through monthly health information sessions and immunizations.

In addition, Dr. Rocafort is also a Board Certified Ambulatory Care Pharmacist and active member of the American Pharmacists Association, National Community Pharmacists Association, and the Phi Delta Chi Professional Pharmacy Fraternity.
